Consent to the processing of personal data for marketing purposes

I hereby agree that the company:

registered in the Commercial Register maintained by the Regional Court in Brno, Section C, Insert 57847
(hereinafter referred to as the “Controller”)

processes my personal data in accordance with Article 6(1)(a) of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) (hereinafter referred to as “GDPR”) and Act No. 480/2004 Coll., on Certain Information Society Services and on Amendments to Certain Acts,

to the extent of: first name, last name, date of birth, residential address, telephone number, email address, for marketing purposes, including direct marketing (in the form of phone calls, sending commercial communications via email, SMS, or other forms of electronic communication, etc.).

I also acknowledge that until the GDPR comes into effect (i.e. until 25 May 2018), the data are provided under Act No. 101/2000 Coll., on the Protection of Personal Data and on Amendments to Certain Acts, as amended.

I am aware of and consent to the fact that the Controller may provide the personal data I have submitted (to the extent specified above) to other processors for further processing of personal data, who process personal data for the Controller and provide guarantees of appropriate and proper processing of personal data and also adequate protection of personal data, including protection through appropriate technical or organizational measures against unauthorized or unlawful processing and against accidental loss, destruction, or damage.

I give this consent for an indefinite period. I am aware of my right to withdraw this consent to the processing of my personal data at any time in writing, by sending an email to: info@borgoagnese.cz or by sending a written request to the Controller’s registered office address, without affecting the lawfulness of processing based on consent before its withdrawal.

I also declare that I am aware of my right to information about the processing of my personal data (in particular, information about the category of processed personal data, the purpose, method and duration of processing, and possible recipients of personal data), the right to request access to my personal data from the Controller, their correction or deletion (especially incorrect, incomplete, or outdated personal data), and the right to file a complaint with the supervisory authority. I acknowledge that from 25 May 2018, I also have the right to the portability of my personal data to a new data controller, and the right to restriction of processing.

I further declare that the personal data I have provided are true and that I have provided them entirely freely, knowingly, and voluntarily.
